D\u00e9pression, anxi\u00e9t\u00e9, pleurs, \u00e9puisement font partie, 15\u00a0jours avant mes r\u00e8gles, de mon quotidien.\u00a0\u00bb<\/em><\/strong><\/p>\n\n\n\n<p class=\"has-text-align-center has-text-color has-link-color wp-elements-ee22c3bbecc1e75b0a769a24f0a63051\" style=\"color:#e37c50\"><strong><em>\u00ab\u00a0Je vois tout en noir et j\u2019ai envie de mourir.\u00a0\u00bb<\/em><\/strong><\/p>\n\n\n\n<p>Ces t\u00e9moignages sont ceux de femmes qui souffrent de TDPM, le <em>trouble dysphorique pr\u00e9menstruel<\/em>. Une instagrammeuse explique en quelques mots&nbsp;: \u00ab&nbsp;C\u2019est comme un syndrome pr\u00e9menstruel, en mille fois pire.&nbsp;\u00bb<\/p>\n\n\n\n<h3 class=\"wp-block-heading has-text-color has-link-color wp-elements-dffcd46f4c2f218f04b0644d6b9d24ef\" style=\"color:#1b7a7f\">Le TDPM, un trouble de sant\u00e9 mentale encore trop m\u00e9connu<\/h3>\n\n\n\n<p>Reconnu en 2013 dans le Manuel diagnostique et statistique des troubles mentaux (DSM, version&nbsp;V), le TDPM est encore si mal connu que l\u2019errance m\u00e9dicale peut durer des ann\u00e9es, parfois plus de 20&nbsp;ans. La Classification internationale des maladies (CIM) le mentionne depuis 2019, contribuant \u00e0 le faire conna\u00eetre, mais la communaut\u00e9 m\u00e9dicale reste si peu inform\u00e9e que le site web de l\u2019association TDPM France propose un guide intitul\u00e9 \u00ab&nbsp;<a href=\"https:\/\/www.assotdpmfrance.fr\/post\/trouver-un-e-sp%C3%A9cialiste\">Trouver un\u00b7e sp\u00e9cialiste qui connaisse le TDPM<\/a>&nbsp;\u00bb. Pour environ 5&nbsp;% des femmes, la p\u00e9riode qui pr\u00e9c\u00e8de les r\u00e8gles est marqu\u00e9e par une d\u00e9pression intense, parfois suicidaire. Le TDPM se distingue du syndrome pr\u00e9menstruel (SPM) par la gravit\u00e9 de ses sympt\u00f4mes psychologiques.<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Les sautes d\u2019humeur sont plus importantes et accompagn\u00e9es d\u2019une tristesse ou d\u2019une d\u00e9pression s\u00e9v\u00e8re.<\/li>\n\n\n\n<li>Irritabilit\u00e9 ou col\u00e8re importante.<\/li>\n\n\n\n<li>Augmentation significative de l\u2019anxi\u00e9t\u00e9 ou de la tension.<\/li>\n\n\n\n<li>Changements physiques&nbsp;:\n<ul class=\"wp-block-list\">\n<li>Changement de rythme du sommeil (augmentation ou diminution)<\/li>\n\n\n\n<li>App\u00e9tit&nbsp;: suralimentation ou fringales<\/li>\n\n\n\n<li>Baisse d\u2019\u00e9nergie<\/li>\n\n\n\n<li>Sensibilit\u00e9 ou gonflement des seins<\/li>\n\n\n\n<li>Douleurs articulaires ou musculaires<\/li>\n<\/ul>\n<\/li>\n<\/ul>\n\n\n\n<p>Ces sympt\u00f4mes surviennent de mani\u00e8re r\u00e9p\u00e9t\u00e9e durant la phase pr\u00e9menstruelle du cycle et disparaissent rapidement au d\u00e9but des r\u00e8gles. Ils ont aussi un retentissement n\u00e9gatif sur le travail ou le fonctionnement social.<\/p>\n\n\n\n<h3 class=\"wp-block-heading has-text-color has-link-color wp-elements-837943fdd667808a90dd7c5b0af3e188\" style=\"color:#1b7b80;font-size:28px;font-style:normal;font-weight:400\">Crit\u00e8res de diagnostic du TDPM selon le DSM-V<\/h3>\n\n\n\n<ol style=\"color:#1b7b80;background-color:#a4cacc66;font-style:normal;font-weight:200;list-style-type:upper-alpha\" class=\"has-text-color has-background has-link-color has-medium-font-size wp-block-list wp-elements-bf732f4e776c933bad7ba9ba8ebb9e41\">\n<li style=\"font-size:17px\">Au cours de la majorit\u00e9 des cycles menstruels, au moins cinq des sympt\u00f4mes suivants doivent \u00eatre pr\u00e9sents dans la semaine qui pr\u00e9c\u00e8de les r\u00e8gles, commercer \u00e0 <em>s\u2019am\u00e9liorer<\/em> dans les premiers jours qui suivent le d\u00e9but des r\u00e8gles et devenir <em>minimaux<\/em> ou absents dans la semaine apr\u00e8s les r\u00e8gles.<br> <\/li>\n\n\n\n<li style=\"font-size:17px\">Au moins un des sympt\u00f4mes suivants doit \u00eatre pr\u00e9sent&nbsp;:\n<ol class=\"wp-block-list\">\n<li>Labilit\u00e9 \u00e9motionnelle marqu\u00e9e (p.&nbsp;ex. mouvements d\u2019humeur, brusque sentiment de tristesse, envie de pleurer, hypersensibilit\u00e9 au rejet).<\/li>\n\n\n\n<li>Irritabilit\u00e9 marqu\u00e9e ou col\u00e8re ou augmentation des conflits interpersonnels&nbsp;?<\/li>\n\n\n\n<li>Humeur d\u00e9pressive marqu\u00e9e, sentiments de d\u00e9sespoir ou autod\u00e9pr\u00e9ciation (id\u00e9es de d\u00e9valorisation).<\/li>\n\n\n\n<li>Anxi\u00e9t\u00e9 marqu\u00e9e, tension et\/ou sentiments d\u2019\u00eatre nou\u00e9, nerveux.<br> <\/li>\n<\/ol>\n<\/li>\n\n\n\n<li style=\"font-size:17px\">Au moins un des sympt\u00f4mes suivants doit \u00eatre pr\u00e9sent, pour atteindre un total d\u2019au moins <em>cinq<\/em> sympt\u00f4mes quand les sympt\u00f4mes des crit\u00e8res&nbsp;B et&nbsp;C sont additionn\u00e9s.\n<ol class=\"wp-block-list\">\n<li style=\"font-size:17px\">Diminution de l\u2019int\u00e9r\u00eat pour les activit\u00e9s habituelles (p.&nbsp;ex. travail, \u00e9cole, amis, loisirs).<\/li>\n\n\n\n<li>Difficult\u00e9 subjective \u00e0 se concentrer.L\u00e9thargie, fatigabilit\u00e9 excessive ou perte d\u2019\u00e9nergie marqu\u00e9e.<\/li>\n\n\n\n<li>Modifications marqu\u00e9es de l\u2019app\u00e9tit, hyperphagie, envie imp\u00e9rieuse de certains aliments.Hypersomnie ou insomnie.<\/li>\n\n\n\n<li>Sentiment d\u2019\u00eatre d\u00e9bord\u00e9 ou de perdre le contr\u00f4le.<\/li>\n\n\n\n<li>Sympt\u00f4mes physiques comme tension ou gonflement des seins, douleurs articulaires ou musculaires, impression d\u2019\u00ab\u00a0enfler\u00a0\u00bb, prise de poids.<br><strong>N.B.\u00a0: Les sympt\u00f4mes des crit\u00e8res\u00a0A-C doivent avoir \u00e9t\u00e9 r\u00e9unis pendant la plupart des cycles menstruels au cours de l\u2019ann\u00e9e \u00e9coul\u00e9e.<\/strong><br> <\/li>\n<\/ol>\n<\/li>\n\n\n\n<li style=\"font-size:17px\">Les sympt\u00f4mes entra\u00eenent une d\u00e9tresse cliniquement significative ou interf\u00e8rent avec le travail, l\u2019\u00e9cole, les activit\u00e9s sociales habituelles ou les relations avec les autres (p.\u00a0ex. \u00e9vitement des activit\u00e9s sociales, diminution de la productivit\u00e9 ou de l\u2019efficacit\u00e9 au travail, \u00e0 l\u2019\u00e9cole ou \u00e0 la maison).<br> <\/li>\n\n\n\n<li style=\"font-size:17px\">La perturbation ne correspond pas seulement \u00e0 l\u2019exacerbation des sympt\u00f4mes d\u2019un autre trouble comme un trouble d\u00e9pressif caract\u00e9ris\u00e9, un trouble panique, un trouble d\u00e9pressif persistant ou un trouble de la personnalit\u00e9 (bien qu\u2019elle puisse se surajouter \u00e0 chacun de ces troubles).<br> <\/li>\n\n\n\n<li style=\"font-size:17px\">Le crit\u00e8re\u00a0A doit \u00eatre confirm\u00e9 par une \u00e9valuation prospective quotidienne pendant au moins deux cycles symptomatiques.<br> <\/li>\n\n\n\n<li style=\"font-size:17px\">Les sympt\u00f4mes ne sont pas dus aux effets physiologiques d\u2019une substance (p.&nbsp;ex. une substance donnant lieu \u00e0 un abus, un m\u00e9dicament ou un autre traitement) ou \u00e0 une autre affection m\u00e9dicale (p.&nbsp;ex. hyperthyro\u00efdie).<\/li>\n<\/ol>\n\n\n\n<p>Il semble que le stress et le fait d\u2019avoir subi des traumatismes tels que n\u00e9gligence ou abus augmentent le risque de souffrir de TDPM.<\/p>\n\n\n\n<p>Selon le site <a href=\"https:\/\/www.esantementale.ca\/Ottawa-Carleton\/Premenstrual-Dysphoric-Disorder-PMDD-Information-for-Patients-and-Families\/index.php?m=article&amp;ID=76638\">eSant\u00e9 Mentale Canada<\/a>, le TDPM est plus probable \u00e0 l\u2019approche de la m\u00e9nopause.<\/p>\n\n\n\n<h3 class=\"wp-block-heading has-text-color has-link-color wp-elements-1b6a0d1255a6f0e2c3776437a2dbbba6\" style=\"color:#1b7a7f\">T\u00e9moignages de femmes souffrant de TDPM<\/h3>\n\n\n\n<blockquote class=\"wp-block-quote has-text-align-right has-text-color has-link-color C\u2019est insupportable, on perd le lien de communication, on se demande si on souffre vraiment, et pourtant la souffrance est bel et bien l\u00e0.<\/p>\n\n\n\n<h3 class=\"wp-block-heading has-text-color has-link-color wp-elements-f7ff50d282170255365c0021ffaf4f98\" style=\"color:#1b7a7f\">Le TDPM, \u00e0 ne pas confondre avec un trouble bipolaire ou borderline<\/h3>\n\n\n\n<p>L\u2019une des raisons de cette m\u00e9connaissance est l\u2019apparente similitude entre le TDPM et le trouble bipolaire. Les crises d\u00e9pressives succ\u00e8dent \u00e0 des p\u00e9riodes d\u2019humeur normale ou euphorique.<\/p>\n\n\n\n<p>Le <a href=\"https:\/\/www.youtube.com\/watch?v=7gr6ClnCmmM&amp;ab_channel=NCMH%28NationalCentreforMentalHealth%29\">NCMH (centre national britannique de la sant\u00e9 mentale)<\/a> \u00e9tudie les diff\u00e9rences entre ces deux affections, car le traitement n\u2019est pas le m\u00eame. Si les antid\u00e9presseurs sont l\u2019un des traitements les plus efficaces dans le TDPM, ils sont contre-indiqu\u00e9s dans le trouble bipolaire.<\/p>\n\n\n\n<p>Sympt\u00f4mes communs au TDPM et au trouble bipolaire&nbsp;:<\/p>\n\n\n\n<ul style=\"font-style:normal;font-weight:200\" class=\"has-medium-font-size wp-block-list\">\n<li>Irritabilit\u00e9<\/li>\n\n\n\n<li>Sautes d\u2019humeur<\/li>\n\n\n\n<li>Humeur d\u00e9pressive<\/li>\n\n\n\n<li>Anxi\u00e9t\u00e9<\/li>\n<\/ul>\n\n\n\n<p>Le TDPM peut aussi \u00eatre confondu avec un trouble de la personnalit\u00e9 limite (borderline), et l\u00e0 encore, le NCMH met en garde car les traitements sont diff\u00e9rents.<\/p>\n\n\n\n<h4 class=\"wp-block-heading has-text-color has-link-color wp-elements-699966f47d2ea997fc70211c89b2b30c\" style=\"color:#1b7a7f\">Comment distinguer TDPM, trouble bipolaire et trouble borderline&nbsp;?<\/h4>\n\n\n\n<div class=\"wp-block-group has-medium-font-size\" style=\"font-style:normal;font-weight:200\"><div class=\"wp-block-group__inner-container is-layout-constrained wp-block-group-is-layout-constrained\">\n<p>Le moment o\u00f9 les sympt\u00f4mes se manifestent<\/p>\n\n\n\n<p><span style=\"text-decoration: underline;\">Trouble bipolaire<\/span>&nbsp;: p\u00e9riodes distinctes<\/p>\n\n\n\n<p><span style=\"text-decoration: underline;\">TDPM<\/span>&nbsp;: dans la majorit\u00e9 des cycles, pendant la semaine qui pr\u00e9c\u00e8de les r\u00e8gles, s\u2019am\u00e9liorent dans les premiers jours des r\u00e8gles, puis s\u2019att\u00e9nuent ou disparaissent compl\u00e8tement dans la semaine qui suit les r\u00e8gles.<\/p>\n\n\n\n<p><span style=\"text-decoration: underline;\">Trouble borderline<\/span>\u00a0: apparaissent au d\u00e9but de l\u2019\u00e2ge adulte et se manifestent dans diverses situations.<br> <\/p>\n<\/div><\/div>\n\n\n\n<h3 class=\"wp-block-heading has-text-color has-link-color wp-elements-f7a33d521d3cd4f3b8d40da62192c2a0\" style=\"color:#1b7a7f\">Caract\u00e9riser les sympt\u00f4mes du TDPM<\/h3>\n\n\n\n<p>Le caract\u00e8re cyclique des sympt\u00f4mes est typique.<\/p>\n\n\n\n<div class=\"wp-block-group has-text-color has-link-color wp-elements-5d657afbe58029fcf552cd1a1c429586\" style=\"color:#e37b57\"><div class=\"wp-block-group__inner-container is-layout-constrained wp-block-group-is-layout-constrained\">\n<p class=\"has-text-align-center has-text-color has-link-color wp-elements-e0730bcba082ef20fa26e5d677d68be0\" style=\"color:#e37b50\"><em>\u00ab\u00a0Mon entourage qui ne comprenait plus du tout pourquoi parfois j\u2019\u00e9tais en pleine forme et \u00e0 d\u2019autres moment tr\u00e8s mal, incendiaire et d\u00e9sagr\u00e9able ou en pleurs pour rien\u00a0\u00bb<\/em><\/p>\n\n\n\n<p class=\"has-text-align-center has-text-color has-link-color wp-elements-e2dee76bdb5ce36a3fce0c3db56258fd\" style=\"color:#e37b50\"><em>\u00ab\u00a0Comme un trop plein qui explose, puis retombe d\u2019un coup. (\u2026) Puis \u00e7a passait mais les crises \u00e9taient tellement fortes que j\u2019\u00e9tais \u00e9puis\u00e9e, vid\u00e9e et anxieuse de la prochaine\u00a0\u00bb<\/em><\/p>\n<\/div><\/div>\n\n\n\n<p><\/p>\n\n\n\n<p>Il est recommand\u00e9 d\u2019enregistrer et de suivre les sympt\u00f4mes sur plusieurs cycles cons\u00e9cutifs pour faciliter le diagnostic de TDPM.
